For the consideration described herein The Dog Obedience Academy agrees to provide the services indicated below for OWNER and OWNER’S dog (“dog”). OWNER represents that he/she is the legal owner of the dog described as:

Payable in advance, the Dog Obedience Academy agrees to provide basic obedience training for OWNER’S dog. This training consists of and is not limited to basic on leash behaviors including sit, stay, down, sit-stay, down-stay, leave it and loose leash walking. Dog problem behaviors that are to be worked on while attending the Dog Obedience Academy will be written out as an addendum to this contract and signed by both parties.

THE Dog Obedience Academy IS NOT LIABLE FOR DOG’S PERFORMANCE Although THE Dog Obedience Academy will use all reasonable care to train owner’s dog, THE Dog Obedience Academy cannot guarantee that the dog will respond adequately to commands at all times. OWNER agrees that THE Dog Obedience Academy will not be liable for any damage or loss resulting from the dog’s failure to respond to any command(s) or behavior modification for which the Dog Obedience Academy trained the dog. THE Dog Obedience Academy agrees to train OWNER’S dog in reasonable manner. However, the effectiveness of completed training program depends upon many factors over which THE Dog Obedience Academy has no control. Such factors include, but are not limited, to, the future health of the dog, the frequency and effectiveness of training reinforcement which the OWNER must give the dog and the degree of respect which the OWNER instills in the dog. Should the dog injure any person, or should the dog’s failure to respond to any command(s) result in damage to the property or person of any person (including the OWNER), OWNER agrees to absolve THE Dog Obedience Academy from any and all obligations and to pay any such damage to such person, OWNER further agrees to hold THE Dog Obedience Academy harmless from any and all such claims. The owner shall provide documentation of current up to date vaccinations including Rabies, DHLPP, Bordatella and Corona. The Owner agrees to read the handouts given at the initial meeting with the Owner and The Dog Obedience Academy, before picking up their dog. Any questions or concerns will be gladly addressed when the dog is picked up. The Owner agrees that at any time the Dog Obedience Academy deems that the Owner?s dog needs veterinary care, if urgent. shall be immediately taken to the local veterinary clinic. If veterinary care is not deemed urgent, the owner will be notified first. The owner agrees to pay any associated vet bills. This written CONTRACT constitutes the entire agreement between The Dog Obedience Academy and OWNER and may not be changed except by a writing signed by The Dog Obedience Academy and OWNER.
